The rising incidence of thrombosis post-coronavirus disease 2019 (COVID-19) places an increased burden on health care systems, as even young patients with prior asymptomatic severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) infection can suffer from delayed catastrophic thrombosis. Mechanistically, post-COVID-19 thrombosis may arise from persistent thromboinflammation, in turn arising from chronic hypercoagulability, endothelial dysfunction, inflammation, neutrophil extracellular trap formation, and platelet hyperactivation, which are detectable in some patients who have recovered from COVID-19. Here, we present radiological and histopathological evidence of chronic, large vessel vasculopathy associated with recurrent post-COVID-19 thrombosis in a young male.
